Matt Stanley c0ffd6d4c5 Support guarded mod address adapters and atomic extended memory cards
Extract the audited adapters and card transport onto master without the held selector/menu stack. Keep adapters and codecs tied to mod activation, use overlay ABI 33, retain rejected-load protection on codec registration, and refuse extended-card codecs in netplay before frame zero. Preserve default atomic save publication and guest-visible write failures.

/* Optional game-owned metadata after the standard 128KiB image. The serial
* device and raw export see only the standard image. A missing codec preserves
* a loaded opaque tail unchanged; its title-specific binding may become stale.
* Callbacks run on the emulation thread and must not re-enter card I/O.
* load: zero accepts, negative blocks flushing until a successful reload.
* save: return tail byte count (zero = raw card), negative aborts the flush.
* Both callbacks are required. Reset before each mod session's activation.
* Selecting a codec blocks existing cards until initialization/reload has
* successfully validated them with that codec's load callback.
* The codec must define native save fallback, slot identity and restore policy;
* this transport cannot infer those from an opaque tail. */
