Matthew Stanley a017edbb7e crash_trace: stamp build identity (git rev + build time) into reports ...

A user-submitted crash report (issue #1) had no way to identify which build
produced it. Embed PSX_BUILD_REV (git describe, computed in runtime.cmake at
configure time; "unknown" on failure) plus __DATE__/__TIME__ as a "build" field
in every report, so any user crash can be correlated to an exact build.
