Controller: l3=leftstick / r3=rightstick defaults (map rows were missing
entirely). Keyboard: T/Y defaults. Beetle oracle keyboard: T/Y. Shipped
input.ini templates + README updated. Validated on Ape Escape (user-
confirmed L3/R3 on real pad).
