Kaoru0822 89252f1f35 fix(evidence): report missing evidence files instead of permission advice
Reading a nonexistent Evidence bundle or snapshot, or exporting into a
directory that does not exist, failed as reason "io" with "Evidence
file could not be accessed. Check file permissions", and with no path.
In `rea compare` the caller could not tell which of the two files
failed.

Map ENOENT and ENOTDIR to a new "missing" reason with read- and
write-specific advice. Every EvidenceFileError now carries the resolved
caller path in its projected details. Permission and other I/O failures
keep the existing advice.

Kaoru0822 cf96ceb211 fix(evidence): name the failed constraint when a bundle is rejected
evidence-import, evidence-export, compare and the MCP bundle tools gave
every rejected file the same message: "Evidence is invalid or has
changed. Recreate or re-import it". That included a single Evidence
record, which is what every command prints, so `rea compare a.json
b.json` on two command results looked like tampering.

Report a single record as such and point to export_evidence_bundle.
Locate the first schema issue, and keep thrown constraint messages such
as a semantic identifier mismatch. EvidenceIntegrityError now accepts
the userMessage option that other typed errors already use.

Dery Ferdika bfbfc7330a fix(domain): reject excessive JSON depth with actionable diagnostics (#1029)
* fix(domain): bound JSON depth and oversized payloads at parsing boundaries

jsonValueSchema re-walked hostile input through z.json()'s recursive core, so a deeply nested payload from an MCP tool argument, inline evidence, provider result, or keyed archive threw RangeError: Maximum call stack size exceeded instead of a validation issue. The schema now runs an iterative depth guard first and keeps z.json() for full JSON semantics behind it, which also turns cyclic values into validation errors. The 1000-edge limit stays far below the range where the recursive core overflows its stack (measured between 2000 and 3000 on Node 24) while leaving room for legitimately deep target-derived trees such as compiled NIB view hierarchies, and the guard narrows with a typed record check instead of any or unchecked casts. The non-recursive JSON Schema advertisement stays intact on both the public and standard projection paths. Two payload sizes get explicit budgets tied to their producer constraints: web text artifacts reject beyond the 16 MiB source budget the web tooling already advertises, and native UI screenshot base64 beyond the 64 MiB helper output budget. Regenerated product catalog, skill catalog digest, and completion-ledger manifest follow the schema-boundary change.

* fix(validation): preserve depth diagnostics across process inputs

Kaoru0822 5acdb9dc6f chore: merge main into feat/dylib-resolution
Resolve conflicts against main:
- README.md: take main's simplified English README, which no longer has
  the tool-family table.
- README_ja.md: take main's reworded table and count
  trace_dylib_resolution in the artifact graph row (5 -> 6).
- Skill: the references gain the trace_dylib_resolution paragraph, so
the version moves to 29 (main already uses 28).
- Regenerate the product catalog, conformance manifest, skill digest and
package metadata (135 tools). The MCP tool catalog is now built rather
than committed (#997), so it is removed here.

DLME2024 96a46ada0a fix(javascript): bound semantic node projection for resource safety (#966)
* fix(javascript): bound semantic node projection for resource safety

analyze_javascript_application exhausted the heap or failed serialization on
ordinary inputs. A small extracted tree crashed outright, and a larger
application tree failed at both the default heap and an 8 GB heap.

Cause: the semantic projection emits one node per AST expression, call site,
binding and property slot, with no ceiling. A five-file tree already yields on
the order of 170,000 nodes, and the whole result is serialized after the object
graph is built and validated. Result ceilings were deliberately removed upstream,
so nothing bounded this growth.

Apply two resource-safety ceilings and report them through the graph's own
coverage contract rather than dropping data silently:

- SEMANTIC_GRAPH_NODE_CEILING bounds retained nodes per tree (100,000, roughly
300 MB serialized, below V8's 512 MB string limit).
- SEMANTIC_GRAPH_FILE_NODE_CEILING bounds each source file (20,000) so one
bundled library cannot starve the application's own modules.

Exhausting either marks coverage truncated with a partial status and records the
ceiling in coverage.limits. A small application stays complete and untruncated.

Verified: a previously-crashing application tree now returns Evidence in under a
minute, while a small tree stays complete. Existing tests and check:fast pass.

* fix(javascript): report semantic graph truncation accurately

* fix(artifacts): accept the bundle subject when extracting an app bundle

For a .app target the session target is the bundle's executable, while the
artifact provider reports the bundle itself as the subject. The subject
check exempted inventory_artifact and inspect_artifact but not
extract_artifact, so extracting any .app failed with execution_failure after
the output tree had already been written, and the output path was never
reported.

* refactor(binary): rename artifact bundle subject helper

isArtifactInventorySubject also covers extract_artifact for .app
bundles, so name it for the shared bundle-subject exemption.

* fix(javascript): resolve bare source-map and worker URLs next to the script

A sourceMappingURL like `main.js.map` (the default of webpack, esbuild,
