refactor(cleanup): share failure composition and retain native UI owners
Validation: integrated changes passed formatting, typecheck, lint, resolved module-boundary verification and git diff --check. Tests and real-provider workflows were not run.

fix(session): retain failed provider cleanup for lifecycle retries (#1260)
Retire inactive clients until cleanup succeeds and block replacement startup
while owned resources remain. Preserve startup/cancellation failures alongside
cleanup diagnostics and retain explicit document choices across retries.

fix!: restore caching, preserve failures, and verify fixture claims (#1211)
Restore profile-bound Ghidra snapshot reuse, preserve direct-analysis results through cleanup failures, and grade configured agent fixture claims against returned evidence.
Register the compiled agent-evaluation fixture entry for static analysis.
