* fix(mcp): escalate transient failures before asking for a restart
Give dropped connections and startup failures one bounded retry, then a
scripted handoff to the user. Point timeout recovery at a named request
field or REA_* setting instead of telling agents those limits are fixed.

* fix(process): stop unexpected command failures from recommending a retry
The catch can already have launched the target, and the projection stays
non-retryable. Tell the user to check the inputs and run doctor instead.

* fix(mcp): limit retries to safe calls and recommended settings
A dropped connection may already have changed the target, and a bad
catalog rejects the same startup every time. Retry only a repeatable
call, and change a setting only when the remediation recommends it.

* test(mcp): match recovery guidance across wrapped lines
Startup handoff capitalization and markdown wrapping still carry the
same sentences. Compare the prose with whitespace collapsed.

Tag physical read outcomes at the owning boundary and preserve opaque caller cancellation reasons through JSON pipelines and reference imports. Share named bounded-read options across consumers and retain failed cleanup ownership for retry.
Closes#5686
Retain process input and unexpected failure diagnostics through the shared error projector. Preserve filesystem causes and firmware validation issues, and mark unchanged invalid requests nonretryable.
Use typed stat settlement and parse unknown command failures without losing null or undefined rejection causes.

Add shared, typed input-contract and timeout-state recovery guidance. Normalize SDK input failures within the actual MCP wire budget, preserve full diagnostics and protocol metadata, and retain original Evidence when normalized delivery would exceed the budget.
Fixes#5431.
Keep failed JSON read handles in the existing server resource scope and fail admission until cleanup succeeds. Thread cancellation through every process CLI input read and replace timer-driven cancellation checks with a shared first-read gate.
Classify authenticated standalone Evidence as wrong-kind input while preserving tampered-record integrity failures and actionable messages across ledger, CLI, and MCP consumers. Record unrelated CS_RESTRICT process neighbors as unverified without weakening related-process ownership checks. Update stale ADB fixtures for the actual shell protocol.
Validated focused CLI, MCP, filesystem, ownership, and error projection tests; static, docs, and duplication gates pass. The host-level CS_RESTRICT load reproduction was not run. Refs #2050, #2035, #2064.
