morluto 153017d750 fix(reconstruction): preserve residual replay uncertainty ...

Keep overall verification unknown when an unchanged process comparison cannot
prove the absence of divergence. Preserve independently complete dimensions
and known changes. Let the named comparison schema own status consistency.

Validated through stdio MCP comparison and verification, plus domain tests.
