morluto 45b6a7fe8f fix(native): distinguish proven pre-launch failure from unknown target ownership ...

Emit explicit target-creation facts only when known. Invalid post-launch process handles remain unknown and retain cleanup ownership.

Refs: #3185

Validation uses bridge fakes and provider-boundary tests; no real LLDB target launch was performed.
