Pull swiotlb updates from Konrad Rzeszutek Wilk:

 - minor fixes (rate limiting), remove certain functions

 - support for DMA_ATTR_SKIP_CPU_SYNC which is an optimization

In order to build the documentation, use ``make htmldocs`` or
``make pdfdocs``.

There are various text files in the Documentation/ subdirectory,
several of them using the Restructured Text markup notation.
